SilverArc Apn an Integrative Innovative System of Health offering affordable monthly membership plans.  This program is designed to provide patients new and established with access to quality and affordable integrative care.  This integrative system of health fuses Western and Eastern philosophies to treat the whole person.

 

What does that mean? This 3-step process includes evaluation, care plan and follow-up which is uniquely designed for each patient.  This process is utilized for all patients regardless of their initial symptoms for care.  We have developed this process because we are passionate about helping people stay healthy, rather than needing to get healthy.

Wellness & Prevention

    • Initial evaluation - obtain Full medical, Psychological, Emotional, Substance Use, and Environmental History, Basic Physical Examination (excludes gynecological exam)

    • Initial diagnostic labs

  • Chronic Disease Management – High Blood Pressure, Diabetes, Blood Pressure, Etc

Non-Member Services

Non- Member services are the same as those with memberships with the exception of Member Benefits.

 

$50 Non - refundable deposit that is credited to the visit cost

WHAT IS THE INTAKE FEE FOR?

  The intake fee includes the cost of basic lab work and will be administered at the initial consultation (1 hour).

 

DO YOU SEE CHILDREN?

   Not at this time, our office isn't properly equipped.

 

DOES EVERY PATIENT RECIEVE THE SAME TREATMENT?

  No, services will vary from patient to patient.

 

CAN I CANCEL MY MEMBERSHIP?

  An initial 12-month membership is required.  Once 12 months have passed, a 30 day written notice will cancel your membership.  Why a 12 month obligation?  We believe that it is easier to stay well than get well and sometimes it can take several months for patients to get and stay well.

 

WHAT DO I DO IF I NEED TO CANCEL AN APPOINTMENT?

  The best way to cancel an appointment is to give us a call at 702-586-5060.  A 24 hour notice is required or a $25 fee will be charged. 

 

CAN I EXPECT ADDITIONAL COSTS?

  Due to the specialized knowledge, or intense time required, care such as but not limited to: Addiction Care, Supplies & Injections.

 

 

DO YOU TAKE INSURANCE?

   A few insurances have added us to their panels. Please call the office for more information

DO I STILL NEED INSURANCE?

  OUR program is NOT an insurance plan. Our program is built around assisting you to maintain your health. Insurance is still need to for those unforseen events that occur and you end up in the hospital or need special care.

CAN MY FEES BE REIMBURSED BY MY INSURANCE COMPANY?

  In some cases, you can submit a receipt for your membership fees or lab/medication costs.  Some insurance companies will apply these payments toward a deductible.

 

DO YOU ACCEPT HEALTH SAVINGS ACCOUNT (HSA)?

SilverArc APN as a health care provider is authorized to accept health saving account (HSA) payment. 

 

A health savings account (HSA) is a tax-advantaged medical savings account available to taxpayers in the United States who are enrolled in a high-deductible health plan (HDHP). The funds contributed to an account are not subject to federal income tax at the time of deposit.


Because we accept HSA payments you can receive your care without additional out of pocket payments. We will provide a superbill you can submit to your insurance company in the hope it can be used to meet your deductible requirement.

 

 

WHAT IF I NEED TO BE REFFERED TO A SPECIALIST?

  SilverArc APN will refer you to a specialist when you need care beyond the scope of our services. We will work with you to select the specialist you feel is right for you.

 

 

ARE THERE MEDICATIONS SILVERARC APN DOESN'T PRESCRIBE?

  The short answer is no but there are limitations. We do not prescribe opiates, benzodiazepines, most psychiatric medications or stimulant medications on a routine basis. We believe there are far better means of dealing with pain, and stress than the use of mood or mind altering medications.  

 

As a family nurse practitioner we choose not to initiate the routine use of antipsychotic, or mentally stimulating medication. 

 

In the process of assisting you to stop a substance you are not able to stop we will prescribe a number of medications for a short term and in limited quantities. 

 

We are able to offer buprenorphine/naltrexone treatment to a limited number of patients as part of our Medication Assisted Treatment for substance use program

 

 

SILVERARC APN HAS THE RIGHT TO REFUSE MEMBERSHIP.
